    OUT-OF-POCKET EXPENSES REFER TO THE PORTION OF THE BILL THAT THE INSURANCE COMPANY DOESN’T COVER AND THAT THE INDIVIDUAL MUST PAY ON THEIR OWN.
WHEN YOUR OUT OF POCKET IS MET, THE INSURANCE WILL PAY A FIXED PERCENTAGE, WHICH IS NORMALLY 100%.

    THE INSURANCE COMPANY BASES VISITS ON WHAT THEY BELIEVE TO BE MEDICALLY NECESSARY, ALSO KNOWN AS COVERAGE DETERMINATION POLICY. TO HELP THEM DETERMINE THE VISITS THEY WILL USE THE FOLLOWING INFORMATION:
 FREQUENCY AND DURATION OF VISITS DETERMINED BY THE PHYSICIAN & THE EVALUATION OF YOUR VISIT BY THE PHSYCIAL THERAPIST.

THE INSURANCE COMPANY ALLOWS A CERTAIN AMOUNT OF VISIT PER YOUR PLAN CONTRACT.

    MEDICARE VISITS

    
$3000 THRESHOLD COMBINED WITH SPEECH THERAPY AND OCCUPATIONAL THERAPY. UPON REACHING THE THRESHOLD OF $3000 A MEDICAL REVIEW PROCESS WILL BE INITIATED BY MEDICARE. AT THAT POINT IF MORE ARE NEEDED WE WILL PETITION FOR YOUR CONTINUED CARE BASED ON OUR CLINICAL RECORDS AND THAT OF THE PHYSICIAN

BASED ON A DOLLAR AMOUNT. ONCE THAT IS MET PLEASE LET US KNOW THE FOLLOWING:

    • PROVIDE US WITH YOUR P.I.P. LETTER. P.I.P. STANDS FOR PERSONAL INJURY PROTECTION. THIS LETTER SHOWS THAT YOUR AUTO INSURANCE WILL NO LONGER PAY. WE WILL SEND THAT LETTER TO YOUR HEALTH INSURANCE SO THAT THEY WILL PAY (GIVEN THAT YOU PROVIDE US WITH YOUR HEALTH INSURANCE). WE NEED TO KNOW WHAT HEALTH INSURANCE YOU HAVE TO MAKE SURE WE ACCEPT IT, AND IF WE DO ACCEPT IT, WE NEED TO KNOW IF IT REQUIRES AUTHORIZATION FOR PHYSICAL THERAPY.

WE HAVE THE ABILITY TO AUTHORIZE THROUGH TRICARE’S WEBSITE FOR MOST CASES. HOWEVER SOMETIMES THE PATIENT NEEDS TO OBTAIN AUTH THROUGH THEIR PCP, VETERANS AFFAIRS OR HEALTHNET.

    Do I need a referral to start Physical Therapy?
    Utah allows patients direct access to physical therapy services and most insurances do not require a referral.
    What is Physical Therapy?
    Physical therapy is care that aims to help relieve pain and help you move and function better.
    What Is A Therapy Visit Like?
    Your therapy visit will include activities specifically designed to treat your condition. This will almost always include a targeted exercise routine.
    How can I know if physical therapy will benefit me?
    In working with you and your doctor, we can discuss your options and determine if physical therapy is right for you.
    Can I choose my own physical therapy?
    As a patient you have the right to choose where you want to go for physical therapy.

    What To Expect With Your First Visit?
    After your physical therapist talks to you about your condition, they will then perform an examination. Your PT will focus on measuring impairments that may be causing your problem or that may be affected by your injury. During your first visit, your physical therapist will seek to gain a thorough understanding of your specific condition. They will do this by an interview and an examination. The interview will allow you a chance to explain how your condition is affecting your life. The examination will look at impairments that may be causing your symptoms and may include measurements of range of motion, strengthen, function, balance as well as neurological screening tests. The therapist will then begin treatment on the first visit.
    What should I bring with me to my first appointment?
    Please arrive 15 minutes early to check in with the front office staff and to complete paperwork. You will also need to bring your insurance information and a doctors referral (if you have one, but it is not required).
    What should I wear on my first visit?
    Comfortable, loose fit clothing that allows the therapist access to any problem areas. A comfortable pair of sneakers is also recommended.
    How long will each appointment last?
    Your appointments will typically last any where from 45 minutes to 90 minutes.
    How long will I have to come to PT?
    Each patient’s diagnosis is different but you can expect anywhere from 4 -12 visits on average. Your plan of care and number of visits will be determined during your initial evaluation.
    Do you have a wait list?
    We do not have a waiting list. We will schedule your appointment as soon as it is convenient for you and usually within two to three days. We can often see patients same day if needed.
    How many times a week will I have to come?
    On average, two to three times per week.

    Do you take walk ins?
    We do, pending insurance approval, some insurances require pre-authorization for physical therapy.
    Will physical therapy hurt?
    Physical therapy is meant to relieve your pain. However, as you restore range of motion and strength you may experience some temporary soreness. We will do everything we can to make your therapy experience as comfortable as possible.
